 Job Description
Centacare has an exciting opportunity for three Client Support Officers to join their Community Care Service Team based in Rockhampton. Two full time postions of 76 hours per forntight and one part time postions 52 hours per fortnight.
The successful applicants will work with the Coordinator in the development of an innovative Service Delivery Model reflecting the new Aged Care reforms to ensure Centacare has positioned itself to provide high quality Consumer Directed Care services in the future.
Community Care Services provide a range of basic maintenance and support services to eligible clients living in the community setting who would otherwise be at risk of premature or inappropriate long term residential care; clients include older frail people and younger people with moderate to severe disabilities, and the unpaid carers of the people within the program.
More specifically, your duties will include:
Process client assessments and referrals, conduct intake and review as per program guidelines and protocols in consultation with the Coordinator
Liaise with necessary stakeholders regarding client care
Develop and review client care plans using the ONI screening tool, 6 monthly or annually in consultation with client/carer/advocate/coordinator as per program guidelines
Provide services in line with program guidelines
Be familiar with a range of HACC & CACPs services available at Centacare and other services in the Community that may assist clients and carers
