Centacare has an exciting opportunity for a Community Care Services Coordinator to join the team in Blackwater on a part-time basis, working 50 hours per fortnight.
As part of Centacare's Community Care Services Team, this position works under the general direction from senior employees however the application of a high level of knowledge and skills is required to achieve results in line with the organisation's goals. This position works within established work practices however generally this level requires involvement in establishing organisational processes and procedures. In addition employees will be required to set priorities and monitor work flows in their area of responsibility and interpersonal skills are required to gain the co-operation of clients and staff.
Main Duties:
* Develop and maintain networks in the community with other Community Agencies and stakeholders in consultation with CCS Coordinator Mackay
* Monitor program outputs and ensure statistical data entry are completed as per funding agreements and Centacare requirements for all HACC and CACPs clients and programs in consultation with CCS Coordinator Mackay
* Monitor and interpret legislation, regulation, program standards and other agreements relating to program delivery, staff and clients in consultation with CCS Coordinator Mackay
* Participate in meetings as required by the role, including Diocesan Working groups as designated by senior management
* Responsible for the day-to-day operations of the Community Services program, to ensure effective and efficient service provision to Centacare HACC and CACPs clients.
* Plan, coordinate, implement and administer the activities of the HACC and CACPs programs in consultation with CCS Coordinator Mackay
* Review client needs to ensure appropriate and efficient service delivery and referral
* Ensure all client records are up-to-date, with up-to-date care plans relating to individual client goals
* Set priorities and monitor team work flow and caseloads in consultation with CCS Coordinator Mackay
